Yakima Neighborhood Health Services Job Title: First Steps Coordinator

 

Minimum Qualifications: Master's Degree in Counseling, Social Work, Nursing, Nutrition or other allied health science. Post-secondary education or management experience or previous work experience at YNHS where applicable skills have been demonstrated may be substituted for education. Skill in customer service. Knowledge of grammar, spelling and punctuation. Skills in data-entry. Ability to speak clearly and concisely. Ability to read, understand, and follow oral and written instruction.

Position(s) supervises: Counselors, Case Managers, Community Workers II

 

Typical Physical Demands and Working Conditions:

Requires sitting for long periods of time, working at a computer terminal. Some bending and stretching required. Ability to lift papers or boxes up to 30 pounds occasionally. Requires corrected vision and hearing to normal range. Manual dexterity and eye-hand coordination required for use of calculators, phones, computer keyboards and other equipment. Requires ability to sustain concentration in a noisy and fast-paced environment. Visitation in client homes year-round is common. Local travel using own car is required.

 

Examples of Duties (this list may not all include all the duties assigned):

  • Train and supervise Counselors, Case Managers, and Community Workers.
  • Work closely with Nursing Supervisor and Nutrition Services Coordinator to identify training and supervision needs of Public Health Nurses and Registered Dietitians providing Maternity Support Services and Case Management.
  • Maintain the guidelines of the Maternity Support Services (MSS) program as defined in the Maternity Support Services Policy and Billing Instructions.
  • Maintain the guidelines of the Maternity Case Management  (MCM) program as defined in the Maternity Case Management Billing Instructions.
  • Work in coordination with other YNHS supervisors to develop and implement YNHS policies, protocols and quality assurance standards.
  • Supervise and evaluate MSS and MCM providers.
  • Maintain and update forms used in MSS and MCM charting.
  • Evaluate and problem-solve denied billings for MSS and MCM
  • Facilitate MSS case conference.
  • Provide the professional services of  his or her specific discipline.
  • Receive patient complaints and compliments. Address issues as needed, and/or direct concerns to appropriate supervisor and/or administrator.
  • Work with other supervisors and administrative staff to address organization-wide issues, including program development, personnel policies, employee evaluations, training needs, safety concerns and quality assurance indicators. Make recommendations and/or decisions for improvement based on the impact to the organization, rather than any single department.
  • Maintain organized, clean, efficient and confidential work area.
  • Represent YNHS in local meetings as requested.
  • Perform related work and other tasks as requested.

All YNHS employees are expected to develop and maintain professional and courteous working relationships with all other YNHS employees, patients, visitors, health profession students, vendors, and volunteers of the organization. In addition, YNHS employees are expected to  provide courteous and efficient service to health and social service agencies working with YNHS patients.
