Title: Case Manager Minimum Qualifications: Associate degree in
behavioral or health sciences or related field and two years closely related
work experience. Qualifying work
experience may be substituted year for year for education. Good attention to detail;
good written and verbal communication skills.

English/Cambodian, English/Vietnamese, and/or ability to sign. Supervised by: First
Steps Coordinator Position(s) supervises: None Typical Physical Demands and Working
Conditions: Requires
prolonged walking and standing,
frequent bending, stooping or stretching. May involve lifting or positioning
patients. Requires corrected vision and
hearing to normal range. Requires full range of body motion including manual
and finger dexterity and eye-hand coordination. Required to work under stressful conditions and with patients who
are often in discomfort and/or pain.
May be exposed to communicable diseases and bodily fluids. Requires ability to sustain concentration in
a noisy and fast-paced environment.
Visitation in client homes year-round is common. Local travel using own car
is required. Examples of Duties (this list may not all

Provide
pregnant/postpartum clients with resources and referrals to become
self-sufficient. ß
Conduct
Maternity Case Management Intake/Assessment to identify client needs. ß
Attend
case conference on a regular basis with other YNHS MSS team members to address
the comprehensive needs of clients. ß
Assist
in development of quality assurance
standards for maternity case management. ß
Develop
and maintain appropriate linkages with community agencies, prenatal providers,
and Maternity Support team members.
Working Relationships: All

profession students, vendors, and volunteers of the organization. In addition, YNHS employees are expected
to provide courteous and efficient
